Applied Imaging occasionally offers onsite biometric screenings for employees.  Employees that complete these screenings either onsite at the health screening event or at their physician's office receive a weekly premium discount on their health insurance.  For those employees with spouses on the plan, they can receive an additional discount if their spouse has a screening done at their physician's office. ​

Please note:

• Fasting for 8 hours before your screening is optional but not required (fasting is no food or drinks except water)
• Screenings will be a finger prick blood panel and your appointment should take approximately 15 minutes
• Screenings will all be in private area with privacy dividers in place to protect your health information. 
• You will get immediate results of screening data - Blood Pressure, Full Lipid Panel, Glucose and BMI

Instructions: 

1. Click the link above based on your location
2. Select Your Screening Date On Top Menu
3. Select Your Screening Time:  Pick one of the open time slots and full in your details
4. IMPORTANT: After each appointment is booked, please close the browser. Otherwise, the original scheduler will remain logged in and have multiple appointments scheduled. Please do not schedule for others under the same log in.

If you work in a different Applied Imaging location or are unable to attend the screening dates, you may complete your requirements in the following ways:

1. Schedule a physical with your doctor and have them submit the alternative screening form on your behalf

3. Employees have the option of scheduling an appointment with the Quest Diagnostic center in their area.  Reach out to Ann Embree for more details.
